Waiver

By signing this Waiver, I acknowledge that I am assuming all risks and taking full responsibility for any personal injury, illness, death, or damage to personal property that may occur during my participation in a space simulation activity, party, experience, or event organized or hosted by Telos Discovery Space Center. I am aware that there are dangers, hazards, and risks associated with space simulations and related activities, such as dark rooms, flashing lights, fog machine smoke, and intense situations. It is strictly prohibited for participants to run, jump, or engage in any behavior that may pose a risk or harm to themselves or others.

All participants must adhere to both posted and announced rules. In addition, I (or my minor child, if participant is below the age of 18) will diligently abide by the safety precautions outlined in video training, written instructions, or verbally communicated guidelines. Whenever there is a need for further understanding, I (or my minor child) will not hesitate to seek clarification. It is important to acknowledge the potential risks associated with space simulations. Thus, I affirm that my (or my minor child's) participation is entirely voluntary, and that I (or my minor child) have been fully informed about all potential risks. Furthermore, I (or my minor child) do not possess any health conditions that may enhance the likelihood of sustaining injuries while participating in this activity.

By signing this agreement, I acknowledge that I will remain legally responsible for any personal actions taken by myself (or my minor child). Additionally, I agree to indemnify and defend Telos Discovery Space Center, along with their officers, directors, contractors, agents, representatives, sponsors, volunteers, employees, and chaperones, against any illness or accident that may occur. This agreement acts as a release, absolving Telos Discovery Space Center from any liability associated with injuries or illness that may arise during space simulations, group activities, birthday parties, or any other activities. Regardless of whether the injuries are due to negligence, I accept full responsibility for any financial obligations resulting from injuries or illness. Furthermore, I consent to being invoiced for any damages or destruction of property at Telos Discovery Space Center caused by myself (or my minor child). By signing below, whether electronically or in writing, I waive all rights to file a lawsuit against Telos Discovery Space Center for any reason.

I hereby give my consent and permission for Telos Discovery Space Center to capture and retain photographic, video, audio, and any other form of electronic recordings of the named participant. It is understood that the ownership of these recordings will be held by Telos Discovery Space Center. I also grant authorization for the use, reproduction, and electronic posting of any of these recordings for any reasonable purpose, as determined by Telos Discovery Space Center, without the need for acknowledgment or compensation. Additionally, I provide my consent to be contacted with relevant information regarding my chosen event, including reminders, booking summaries, promotions, etc. I am aware that I can choose to opt out of receiving these communications at any time.

I hereby consent to the expectation that I (or my minor child) will exhibit appropriate behavior while present at Telos Discovery Space Center. In the event that any member of the staff perceives my (or my minor child's) conduct to be defiant towards established regulations, disruptive, or hazardous in any manner, I will be promptly informed and I (or my minor child) will be escorted out of the simulation. I have carefully perused, comprehended, and consent to all the stipulations and conditions outlined in this WAIVER AND RELEASE, which encompasses the assumption of risk, waiver of liability, and defense and indemnification provisions, thereby relinquishing my legal rights.
